Adult Social Care support people who need extra help due to an illness, disability or frailness. This may be both short and long term.

Adult Social Care also support vulnerable adults to live healthy, independent and fulfilled lives, providing prevention services to help prevent the need to access intensive services; facilitating community empowerment, and supporting those at greatest risk of social exclusion.

For initial enquiries relating to adult social care please call the Berkshire Integrated Hub, Monday - Friday from 8.30am to 5pm

Accessing this service

Eligibility Criteria

The team works with older people, those with a physical disability and those with a learning disability. The team carries out assessments to establish critical eligible needs for adult social care and sets personal Budgets which can be used creatively to meet those needs.

A Personal Budget can be used to purchase a range of services such as day care, respite care, support within the home and support to access community work and leisure.

When you make initial contact with the team, they will ask for some basic details and may give you some initial advice or signpost you to some of the prevention services set up within Wokingham.  You will be allocated a worker who will carry out an assessment for critical need, the assessment will ask you and those who know you well about your needs to help you get the right support. If you have eligible needs following the assessment an Individual Support Plan will be written detailing how you are going to use your Personal Budget to meet those needs. A financial assessment will also be carried out and you may have to contribute towards some of the costs depending on your financial circumstances.
