Optalis: Community Lives - learning disability day service for adults, Wokingham
Providing meaningful day time activities for adults of all ages and abilities. We are also able to support those with complex needs. The Service runs a varied programme of activities with something to suit all. We run in house activities as well as accessing the community. We have our own fleet of vehicles to enable individuals to get out and about during day service hours. Our activities include swimming, bowling, horticulture, sports, cooking, independent living skills, discussion groups as well as music based activities for those with a higher level of need such as interactive story telling, Music & dance, jabadeo and sound about for which our staff have received professional input.
We will conduct an assessment of the individual to identify their needs and wishes. We aim to maintain and develop skills and abilities, and we recognise individuality and celebrate successes. Our clients are assigned a key worker upon entry to the service who will ensure the individual and their parent/carers are supported and happy. We review the service after six weeks to ensure everything is going well, and will make adjustments if required. We then review an individual's service on an ongoing annual basis or sooner should their level of need change.
