Adult Community Mental Health Team (CMHT)
This service is for adults with severe and enduring mental health needs and is provided jointly by Wokingham Borough Council and Berkshire Healthcare NHS Foundation Trust.
All referrals come to us from one place; Common Point of Entry (CPE) team - Tel No 0300 365 2000 (Press option 4). After an initial screening assessment the CPE team will direct you to the most appropriate service to meet your needs, one of which may be the Wokingham Adults Community Mental Health Team (CMHT)
The team is made up of psychiatrists, community psychiatric nurses, clinical psychologists, medical staff, social care practitioners, therapists and support workers. These healthcare professionals will work with you to develop a plan to meet your health and social care needs. You may be allocated a key worker and if you need social care support they will assess your needs. This assessment will help determine whether your needs are eligible for social care and support from us and how best that care and support can meet your desired outcomes. It will also help them decide if you require longer term support and, if appropriate, will set a Personal Budget to meet your needs.

If you are a carer for someone with a severe and enduring mental illness who is known to the Wokingham Adults Mental Health Services (CMHT), please contact the Carers' Lead (Tel. 0118 904 4800). The Carers' Lead can provide information and advice and may carry out a Carers Assessment if appropriate.
